Health Coaching Program Overview

Behavior change can be challenging and the Health Coaching Program offers message-based support and flexibility to help you reach your goal(s) through sustainable lifestyle changes. You'll partner with a coach to receive support, personalized feedback, and ongoing motivation. The program offers guidance on areas such as nutrition, movement, sleep, stress and emotional well-being.

If you are participating in Healthy Choice and required to enroll in the Health Coaching Program, take the next step to connect with a health coach. Your coach will support you to improve your overall well-being. Health coaching is a partnership and together, you will establish sustainable action steps to help you met your short and long-term goal(s).

What is Health Coaching?

A health coach is a professional who cultivates a collaborative relationship with members to guide, encourage and educate them in achieving their personal health and wellness goals. They facilitate lifestyle and behavior modifications. 

These areas include nutrition, physical activity, stress management and overall well-being, empowering individuals to unlock their own potential in their wellness journey.

How does the program work?

Health Coaching is a flexible message-based program to fit your schedule, allowing you to send messages at any time.

You will:

  • Receive personalized support and messages.
  • Receive tips and guidance on areas such as nutrition, movement, sleep, stress, emotional well-being and more.
  • Receive support with personal and Healthy Choice goals that will include communicating with your coach for a minimum of 16 weeks by the program deadline by Sept. 30.

What to expect:

Your goals can be viewed in your Healthy Choice portal. Follow the steps below to start your Health Coaching journey:

Step 1: Enroll through the Healthy Choice portal no later than March 31 to participate for a minimum of 6 months.

Step 2: Connect and message with your health coach in the Healthy Choice portal at least once per week for 16 weeks by Sept. 30.

Step 3: Meet your program and messaging goals, then obtain and report completion of final metrics between Aug. 15-Sept. 30.

Health Coaching Program Rights and Responsibilities

The Employee Health Plan's Health Coaching Program(s) help members manage their overall health through nutrition, fitness, setting goals and monitoring progress. EHP members who participate in the Health Coaching Program(s) have certain rights and responsibilities as outlined below.

Your Rights

  • To know the purpose of your program(s) and understand how it (they) works.
  • To have your personal health information (PHI) relating to the Health Coaching Program(s) shared only in accordance with state and federal laws. 
  • To know your health coach's name, title and how to contact them.
  • To know that messages with your health coach will be stored in a HIPAA compliant platform.
  • To know that calls with your health coach will be recorded for training and quality assurance purposes.
  • To speak to your health coach's supervisor at any time.
  • To receive accurate and timely information from the Health Coaching Program.
  • To receive information about any changes to the Health Coaching Program, including its termination.
  • To stop participating in the program(s) and revoke your consent to have your PHI used for Healthy Choice. 

Your Responsibilities

  • Provide accurate information about your health to your health coach.
  • Answer any questions asked of you or sent to you to complete your program update accurately.
  • Report changes in your Incentive Program to your health coach in a timely way.
  • Respond to outreach attempts of the Health Coaching team in order to keep your program open.
  • Complete and return any forms that are necessary to participate in the Health Coaching Program in a timely manner.
  • Participate in modifying lifestyle behaviors that will help you improve or maintain your health and well-being. 
  • Message your health coach through the Healthy Choice portal to remain an active participant in the program and meet your communication requirement.
